Complimentary Service Consent & Media Release Agreement
This form documents my voluntary participation in receiving complimentary or discounted services from Sage & Honey Wellness in exchange for my agreement to participate in limited promotional activities.
1. Nature of Services
I understand that I am receiving the following service(s) free of charge or at a reduced rate. I acknowledge that this (these) treatment(s) is(are) elective, non-emergent, and provided at the sole discretion of Sage & Honey MedSpa.
2. Promotional Agreement
In exchange for receiving complimentary or discounted services, I agree to the following:Submit a positive Google review about my experience at Sage & Honey Wellness.Allow photos or videos of my treatment, results, or visit to be taken and used by Sage & Honey Wellness for promotional purposes, including but not limited to social media, website, print, and digital advertising.Understand that any photos/videos will be used respectfully and without compromising my privacy, unless I explicitly consent otherwise.
3. Media Use Terms
I grant Sage & Honey Wellness a non-exclusive, royalty-free, perpetual license to use, reproduce, publish, and distribute any images or videos of me taken during or after treatment.I understand that my name or identifying details will not be used unless I provide separate, written consent. I understand that I may request to review or approve the image(s) before publication if desired.
4. Acknowledgment and Consent
I acknowledge and agree that: The service was offered as a goodwill promotion, and I was not required to participate in any promotional activity in exchange for treatment.I have the right to withdraw my media release consent at any time for future use, but it may not apply retroactively to previously published content.I have had the opportunity to ask questions and understand the terms of this agreement.
